How do I refer my student to occupational health? 

Aimed at:  Personal Development Tutors

There is a form to complete. You will find this in the resources section of the tool kit:

It is good practice to complete this with the student present so that they are aware if the content.

Arrange to email and arrange follow up when report is received.
